Diffuus grootcellig B-cel lymfoom (DLBCL)

Diagnose

Standaard op biopt:

  • Immuunhistochemie:
    • CD20 (i.v.m. rituximab)
    • Hans algoritme (CD10, BCL6, MUM1), CD5
  • FISH: translocatie MYC, indien positief dan ook translocatie BCL2 en BCL6 (uitsluiten dubbel of triple hit lymfoom)

Op indicatie op biopt:

  • In situ hybridisatie: EBER (EBV)
  • FISH: translocatie IRF4
  • Moleculaire diagnostiek:
    • B-/T-cel clonaliteitsanalyse
    • Mutatie analyse via NGS panel: o.a. EZH2, CREBBP, MYD88, CD79a/b

Op indicatie op liquor (CNS) en/of vitrectomie (intra-oculair) materiaal:

  • Moleculaire diagnostiek: mutatie analyse via NGS panel: MYD88, CXCR4

Recidief

Standaard op biopt:

  • Clonale vergelijking met eerdere lymfoom, bij interval >5 jaar of verschillend type lymfoom
  • Immuunhistochemie:
    • CD20 (i.v.m. rituximab)
    • CD30 (i.v.m. brentuximab)

Op indicatie op biopt:

  • Immuunhistochemie: CD19 (i.v.m. CAR-T-celtherapie)
