Burkitt lymfoom (BL)

Pathologie (inclusief moleculaire diagnostiek, cytogenetica)

Diagnose

Standaard op biopt:

  • Immuunhistochemie: CD20 (i.v.m. rituximab)
  • FISH: translocatie MYC, indien positief dan ook translocatie BCL2 en BCL6 (uitsluiten dubbel of triple hit lymfoom)

Op indicatie op biopt:

  • In situ hybridisatie: EBER (EBV)
  • Moleculaire diagnostiek:
    • B-/T-cel clonaliteitsanalyse
    • Mutatieanalyse via NGS panel

Recidief

Standaard op biopt:

  • Clonale vergelijking met eerdere lymfoom, bij interval >5 jaar of verschillend type lymfoom
  • Immuunhistochemie: CD20 (i.v.m. rituximab)

Immunofenotypering

Immunofenotypering draagt bij aan de diagnose en stadiëring (uitsluiten / vaststellen extranodale lokalisatie) van rijpe B-celmaligniteiten.

Minimaal panel (B-cellymfoom)

Standaard: 

  • CD45, CD19, CD20, CD5, CD23, CD200, CD10, CD38, IgKappa, IgLambda

Op indicatie / ten behoeve van eventuele karakterisatie:

  • cTdT (ter uitsluiting B-ALL)
